FLORIDA PRESTRESSED CONCRETE ASSOCIATION

PLANT CERTIFICATION APPLICATION

The plant certification program of the Florida Prestressed Concrete Association is designed to certify that each member plant has the ability, procedures and facilities to produce "Standard of the Industry" prestressed concrete. Plant certification is useful as a means of evaluating a producer's in-house quality control program as well as a method of assuring the customer of certified capability.

All plants shall receive regular semi-annual inspections by an independent Registered Engineer retained by FPCA. Each inspection shall be conducted through an entire casting sequence and an in-depth inspection report shall be completed and forwarded to FPCA for final review. The first inspection will be announced, the second, unannounced.

Certification is not automatic. Each in-depth inspection report may disclose deficiencies sufficient to fail the inspection. In addition, each inspection report is subject to final review by the FPCA Engineer or Certification Committee.

William D. Givens, Consulting Engineer, has been retained by the Florida Prestressed Concrete Association to evaluate each inspector utilized by FPCA. All plant certification reports will be on file with FPCA and the Producer Member will be recognized by Certificate and national inclusion in all FPCA publications.

QUALIFICATIONS OF INSPECTOR

1.    Must be Registered Professional Engineer in the state where the plant is located.

2.   Minimum of 10 years experience required in the design and/or production of prestressed/precast
      concrete, or related experience.

3.   Must demonsrate a knowledge of:

                        a.   Design of prestressed concrete                         e.   Materials for prestressed concrete
                        b.   Specifications                                                       f.   Production methods
                        c.   Prestressing                                                          g.   Quality Control
                        d.   Concrete batching, placement & finishing

4.    The inspecting emgineer must be completely independent from the plant management personnel or
        ownership, and must agree to attend an annual workshop conducted by the FPCA Certification
        Committee to work on standards and inspection criteria to develop uniformity and continue
        advancement of the program.
